The bariatric surgery insurance approval process can be challenging. If you are going through your health insurance for coverage of your bariatric surgery, you will have to meet all of their criteria.
These criteria may include:
- Medically Supervised weight loss attempts documented by your physician for a specific number of months (each insurance policy varies on the length of time)
- Body Mass Index requirements (typically a body mass index of 40 or greater without required health conditions or body mass index between 35-39.9 with a diagnosis of one or more obesity-related health conditions, often referred to as co-morbidities)
- Psychological Evaluation
- Nutritional Evaluation
- Medical Clearance
